Dear Certified Disease Manager:
This is the last maintenance cycle for the CDM Certification Program. As was communicated in 2006, the NISPC CDM program will expire on December 31, 2008. After December 31, 2008 CDM Managers will no longer be able to renew, recertify, or maintain certification.
To maintain your current certification you must submit proof of 10 hours of Continuing Education credits in each credentialed area each year, confirm / attest to the currency of your pharmacist license and submit $50 payment for each CDM (or $75 each if after the due date).

CDM Maintenance Fees and Materials may be submitted online at www.nispcnet.org/recertification or by mail BY the above deadlines.
The CDM program will end December 31, 2008.
- CE Guidance: disease specific CE (either all ACPE accredited, or up to five (5) hours of the ten hours each year may be from an accredited health profession accrediting body (CME, Respiratory Therapy, etc) by the above date. The form for recording your CE can be obtained at www.nispcnet.org/recertification.
- Please make sure all communications are sent to the attention of NISPC and clearly state your name and CDM number(s), if known.
Current CDMs are encouraged to seek alternative credentials. A listing of some of those certifications appears below.
Alternative Credentials: The NISPC Board encourages individuals interested in obtaining disease specific recognition to complete a certificate training program offered by one of the professional associations or colleges/schools of pharmacy and to seek certification from one of the multidisciplinary credentialing organizations:
